By using this website, booking an appointment, or messaging us, you agree to these Terms. If you do not agree, please do not use the site.
If you tick a box to agree to these Terms — for example, to receive the first-visit code — you agree to the version in effect on the day you tick it.
Everything on this website — service descriptions, photographs, captions, social media posts, automated replies and messages from us — is general information only. It is not medical advice, not a diagnosis, and not a statement that any treatment is safe, suitable or appropriate for you.
We do not promise results, timelines, or a number of sessions. We do not claim to be better than any other provider. Whether a service is right for you is decided only in person, at your consultation or visit.

Medical weight loss and advanced skin treatments start with a consultation. For every service, your provider decides in person, at your visit, whether it is appropriate to go ahead that day. Any consultation or patch test we require must be completed before the treatment; if you decline a required step, we will not go ahead with that treatment.
Please share health information that may matter for your treatment in person, at your consultation or visit — not by Instagram, Facebook, text or email.
We may decline, stop or reschedule a service for safety or health-suitability reasons, when a step we require has not been completed, when you arrive late (see our Booking Policy), or when a client is abusive to staff or other clients. What happens to your deposit in each case is set out in our Booking, Deposit & Cancellation Policy.
Follow any aftercare instructions we give you at your visit. If you have a question after a treatment, call us at (224) 339-6688 or ask in person. For a medical concern, speak with a qualified healthcare professional; in an emergency, call 911.
Results vary from person to person and are not guaranteed. Treatment photographs are taken only with your separate written consent, and are used publicly only if you separately agree to that in writing, as described in our Privacy Policy.
Booking is governed by our Booking, Deposit & Cancellation Policy, which forms part of these Terms. In summary: a deposit of 30% of the service price holds your appointment and is applied to your total; cancellations more than 24 hours ahead are fully refunded; cancellations inside 24 hours and no-shows forfeit the deposit; if we cancel, you are always refunded in full.
Text messaging is governed by our SMS Terms & Consent. Message frequency varies; message and data rates may apply; reply STOP to cancel and HELP for help. Consent to marketing messages is never required in order to buy anything from us.
Deposits and payments are processed by third-party payment providers. We do not store full card numbers. We never request card numbers, CVV codes or bank details by text, email, or social media message. If you receive such a request claiming to be from us, it is not from us — please call (224) 339-6688.
Billing questions. If you believe a charge is incorrect, please contact us first at (224) 339-6688 or zoyaswellnesscenter@gmail.com so we can review it. This does not limit any right you have with your card issuer or payment provider, or under the law. A balance for services you received that you do not dispute remains due. We do not add fees unless they were disclosed to you in advance and the law allows them.
Promotions in general. Unless an offer says otherwise, only one promotion or discount may be used per visit. Promotions have no cash value and cannot be transferred. We may change or end an offer for future claims; a code already issued stays valid under the terms shown when it was claimed. We may refuse a claim that involves misuse, such as using more than one identity.
First-visit offer. The first-visit code gives 20% off any service on your first visit. It can be used once per client, has no expiration date and cannot be combined with other discounts. We give the code after you share your phone number, email address and birthday (month and day, not the year) and agree to these Terms and our Privacy Policy. When you book online, enter the code in the “Appointment note” field in Square; our staff apply it at checkout. The code has no cash value and cannot be transferred.

The name Zoya’s Wellness Center, our logo and wordmark, the design and layout of this site, and the photographs, text and graphics on it are owned by TOZO LLC or used under licence. You may view and share links to the site. You may not reproduce, redistribute or use our content commercially without our written permission.
If you post a public review or comment about us, you allow us to show or quote it, without changing what it says, with the name you posted under, and you confirm you have the right to post it. We never publish a photograph of you, or a photograph you send us, without your separate written consent, as described in our Privacy Policy. You can withdraw that consent at any time.
Our booking system, payment processors, maps and social media links are operated by other companies under their own terms and privacy policies. We are not responsible for those services, and linking to them is not an endorsement.
We try to keep the site accurate and available, but we do not guarantee it will be uninterrupted, error-free, or that every price, hour or service description is current at all times. Where information here conflicts with what we tell you directly, what we tell you directly governs.
To the fullest extent permitted by law, the website and the general information on it are provided “as is” and “as available,” without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose and non-infringement. We do not guarantee uninterrupted or error-free access to the website.
To the fullest extent permitted by law, TOZO LLC and its members, employees and contractors are not liable for indirect, incidental, special, consequential, exemplary or punitive damages arising from your use of the website. For a claim arising from a specific website transaction, our total liability will not exceed the amount you paid TOZO LLC for that transaction.
Nothing in these Terms excludes or limits liability that the law does not allow to be excluded or limited — including liability for personal injury caused by our negligence, for fraud, for gross negligence or for intentional misconduct — or any right you have under the Illinois Consumer Fraud and Deceptive Business Practices Act. If a specific service needs a consent or release form, it will be given to you separately, in writing, before that service; these Terms do not replace it.
You are responsible for claims a third party brings against TOZO LLC, and the resulting losses, to the extent they are directly caused by your intentional misuse of the website, your unlawful conduct, or your material breach of these Terms. This does not make you responsible for our own negligence or misconduct, or for any liability the law does not allow to be shifted to you.
Before either of us files a lawsuit, the party with the dispute will send the other a written description of it and the outcome sought — to us at TOZO LLC, 1833 Hicks Rd, Suite B, Rolling Meadows, IL 60008, or zoyaswellnesscenter@gmail.com; to you at the contact details you gave us — and we will both try in good faith to resolve it within 30 days. This step does not shorten any filing deadline or limit any right the law does not allow to be limited, and either of us may file sooner if needed to meet a deadline.
These Terms are governed by the laws of the State of Illinois, without regard to conflict-of-law rules. Any lawsuit must be filed in a state or federal court located in Cook County, Illinois, and you and we consent to that jurisdiction, except that either of us may bring a claim in small-claims court where the law allows.
We may update these Terms. The effective date at the top shows the last change. Continuing to use the site after a change means you accept the updated Terms. A change does not apply to a first-visit code already issued, or to a dispute that arose before the change.
If any provision is held unenforceable, the rest remains in force. These Terms, together with the Privacy Policy, SMS Terms and Booking Policy, are the entire agreement between us regarding this website.
